Initial appointment: $180
Individual/Couple/Family: $125/ hour
Group therapy: $55/ 90 minute group
Report writing: $125/hour
Phone consultation: pro-rated for $125/hour
Insurance
I am licensed as a Clinical Social Worker and as a Marriage and Family Therapist in the state of Washington. As a result, most insurance companies consider me an authorized provider of mental health services.
Medical insurance frequently covers psychotherapy for individuals, and in some cases for families and couples. You will need to check your policy to determine which services are included and excluded.
I will provide you with assistance in receiving the mental health benefits to which you are entitled, submitting bills to the companies when that is appropriate, or providing documentation as required by the insurance company, with your release. However, please know that you are responsible for full payment of my fees, not your insurance company. It is your responsibility to contact your insurance company and verify that they cover my services, determine how many sessions your policy allows, and verify your co-payments and annual deductions.
I am a preferred provider for Premara Blue Cross. However, for a number of other large insurers I am considered an ‘out of network provider,’ and the company may deny payment for my services, pay a lower percentage of the fee, and may have a separate deductible for services provided by an out of network therapist. In these cases you will be responsible for the full fee at the time of the appointment. I will provide a billing statement to you, and the insurance company may reimburse a percentage of the fee. It is your responsibility to contact your insurance provider to clarify your coverage.
